2005 Annual Conference Registration

Online Registration Information

  Each person registering for the conference must complete an online registration form. Payment to MASFAA must be received by September 26, 2005. Payment may be made by check or, effective early August, by credit card (Mastercard & Visa). To make payment by credit card you will need your registration invoice number. If payment is received after September 26, 2005, a late fee may be charged.

You are not registered for the conference until payment has been received. Once payment has been applied, a receipt will be emailed to you. Should there be no record of your institution’s payment, you will be asked to pay on-site. If payment is made by check, the check will be held for two weeks to allow ample time for your institution to forward payment. If the payment is not received, your personal check will be submitted for payment.

If you will not be able to attend the conference due to budgetary constraints, consider applying for the MASFAA Conference Scholarship. The scholarship is offered to help defray expenses for MASFAA members who otherwise would not be able to attend the annual conference.

Note: This registration form is for conference registration only; you are responsible for making your own hotel reservation.

The registration form is comprised of multiple pages. The final page of this form will be your invoice. It will include instructions for mailing your payment. You will also receive an email copy of your registration information.

All requests for refunds must be made in writing to the MASFAA Treasurer – Ernesto Monge, University of Wisconsin, 432 North Murray Street, Madison, WI 53706 -1496, ernesto.monge@finaid.wisc.edu. A 100% refund will be granted if the request is postmarked on or before September 26, 2005. Exceptions may be granted upon approval of the MASFAA Treasurer, Vice President, and President. Registration fees for a MASFAA member may be transferred to another MASFAA member. Please contact support@masfaaweb.org to request a transfer.

NOTE: If your conference registration is being paid by someone other than your institution, forward your invoice for payment. Follow-up to ensure payment has been made is registrant’s responsibility.
